Mother nature, capable of destroying anyone, anything, and any event. When you trip over it.
Worst one I saw was in the middle of a night battle, where they called a hold for someone tripping over and mangling their lower leg in a deep forest...
...on a random cinder block. Cue 30 minute pause while they carted the poor guy to the real-life healers. :P
My personal worst- going over a supposedly completely safe barrier, back first onto a boulder. Thank you, metal-plate vest.

I don't really have a personal worst. I knew a lad who messed his leg up good and proper falling off a tank. It was a stationary tank, and had been stationary for a long time. He just climbed up it and fell off.
I've had less than pleasant experiences going through a couple of things, but nothing major. One was an obstacle consisting of a grid of wooden posts and rails, with one foot square holes. There were a few horizontals missing in the middle, making it possible to climb up and squeeze through, or you could climb over the top, but I spotted the missing cross, making a two-point-some foot square hole, low on one side and just went through there, Superman-style. Having a rifle held out in front of me meant I landed on my elbows in the quarry runoff on the far side, and my shins got scraped a bit because I didn't quite clear it. Nothing major. The other was a significant quantity of sheep-**** on the ground. It was dark, so I didn't see it. There wasn't much height to the cover behind me, so I was crawling to avoid being silhouetted against the sky. It had dried on top, so there was no smell as I approached. It's okay, though. I got a shower two days later. Nothing major.
